PLEASE REVIEW THESE GUIDELINES BEFORE REGISTERING FOR AN NCHD VACCINE CLINIC

  • NCHD is currently vaccinating all Phases. Click here to see details.

  • Precedence is given to individuals who live or work in Logan, Morgan, Phillips, Sedgwick, Washington, and Yuma Counties. Other Colorado residents are served as vaccine availability allows. If the vaccine you are given is a 2 dose series, you will need to receive your 2nd dose from NCHD in the same county as your first, about four weeks later.

  • NCHD is currently offering the Moderna vaccine at our clinics which is a 2 dose series. When you receive your first dose, you will receive information to register for your second dose. NCHD will offer the Pfizer and Janssen vaccines as availability allows.

  • Those with a history of severe allergic reactions such as anaphylaxis to any medications may still be eligible to receive the COVID-19 vaccination, but should talk to their doctor first -- please be advised that Northeast Colorado Health Department will not administer the vaccine to these individuals but refer these individuals to a hospital or provider clinic to receive their vaccine.
If you have a history of anaphylaxis to any medication and have made an appointment with Northeast Colorado Health Department for your vaccine, please contact us immediately to cancel your appointment.

  • We reserve the right to cancel appointments if the registrant does not qualify for any one of these guidelines.
