Birth and Death Certificates
Birth and death certificates can be obtained in our Logan, Morgan, Phillips and Washington county offices. Birth certificates are available for Colorado births dating back to 1906. Certain death records may require a waiting period.
Birth certificates are a required form of identification for the following:

Entrance to school

Obtaining a driver's license

Voter's Registration

Obtaining a marriage license

Obtaining a passport

Veteran's benefits

Public Assistance

Register for sports

Baptism of infants

We can provide certificates if the individual was born or died in Colorado.
If the individual was born or died in a state other than Colorado.
NCHD cannot issue birth or death certificates for individuals that were born or died outside of the state of Colorado. You can visit this website to see the procedures of each state.
If you need a birth or death certificate, you can come to any of the four NCHD offices or order online.
You will need:

One approved form of identification
-
Person’s name appearing on the certificate
-
Date and place of birth/ death
-
Parents’ names, along with mother’s full maiden name
-
Purpose for the copy
-
Your relationship to the person for whom the certificate is requested(additional proof may be required.)
Birth certificates cost $20.00 for the first copy and $13.00 for each additional copy when ordered at the same time.
Death certificates cost $20.00 for the first copy and $13.00 for each additional copy when ordered at the same time.
*If the individual died prior to 1975 the death certificate can only be obtained from the county where the death occurred and cannot be ordered online.
