This program is designed to prevent, detect and control cardio cardiovascular disease in adults over the age of 18 and is totally free of charge.
These FREE, yes really FREE, 30-40 minute screenings will identify individuals at risk of heart attack or stroke and consist of:
Cholesterol check with immediate results
Instant blood sugar readings ( those with diabetes, will also receive instant A1C checks)
Blood pressure check
Individual risk assessment for developing heart disease over the next 10 years
Once screened, our NCHD Heath Care Navigator will go over the results, referring them to primary care for treatment if there are any concerns and include follow-ups with assistance in lifestyle and behavioral changes.

Our dedicated staff are available to come and set up at an office site, community center or school.
