Birth and Death certificates, also known as vital records, are legal documents required for such things as entrance into school or settling the affairs of a loved one.
NCHD is the registrar for vital records in Logan, Morgan and Washington counties only, meaning records can only be obtained from us in these three offices. Birth certificates are available for Colorado births dating back to 1906, death certificates must be obtained in the county where the death took place. Specific documentation is required to obtain vital records, please check out the "Frequently Asked Questions" link at left to find out what they are.
To get a certificate for non-Colorado births, use the "Order Online" or "State Contact Info" link at left. To get a certificate for a death that occurred outside Logan, Morgan, or Washington counties, contact the county where the death occurred or the state health department, (303) 692-2200.
